MET Co-Op Membership Agreement<br />
NMI and Member agree as follows:<br />
1.	Enrollment Process.  To become a Member of MET Co-Op, an applicant must sign a completed Enrollment Form and pay the Dues. Once the application is approved, the Member will be issued a Membership Card in-person upon visit to the NMI campus.  NMI has the sole discretion to determine whether an applicant satisfies MET Co-Op membership criteria.<br />
2.	Member Responsibilities.  To receive the 50% tuition discount, member must register and pay the course tuition by the start date of the course.<br />
3.	Membership Term.  A minimum two (2) year initial commitment is required and can be renewed annually after that. Membership begins on the date that the Application Form is approved by NMI (the “Enrollment Date”).<br />
4.	Membership Renewal. After the initial two (2) year membership term, the Member may choose to renew for successive one (1) year periods on the anniversary of the Enrollment Date (the “Renewal Date”).<br />
5.	Membership Cancellation.  A Member may cancel his/her Membership anytime after the initial two (2) year Membership requirement is fulfilled by providing written notice to NMI.  NMI is not obligated to provide any refund for early termination.<br />
6.	Annual Dues.  MET Co-Op Membership Dues (“Dues”) are $199.00* per. The minimum two (2) years of dues ($398.00)are payable in full at the time of enrollment.  Dues for successive one (1) year terms are due on the renewal dates.  Dues will not be pro-rated. Dues may increase or decrease after the first two (2) years of Membership as determined by NMI in its sole discretion. Member will receive written notification of any increase in Dues amount at least thirty (30) days prior to the Member’s Renewal Date.<br />
7.	Refund of Dues.  Dues are non-refundable.<br />
8.	Membership Benefits.  Membership entitles Member to the following benefits:<br />
a.	Tuition Discount of 50% on regularly scheduled Professional Education courses conducted by NMI. Individuals are eligible for the 50% Tuition Discount provided such individuals are registered and have paid their tuition by the course(s)’ start date.  This Discount applies only to courses starting on or after 3 November 2008 and does not include the cost of books and other related course materials.<br />
This 50% Discount does not apply to the following courses:<br />
i.	Evening Captain’s Course<br />
ii.	Advanced Firefighting Course<br />
iii.	Radar Recertification, Flashing Light, RFPNW<br />
iv.	Medical Courses (outside of First  Aid/CPR)<br />
v.	Tutoring<br />
vi.	Long-term work/Co-Op training programs;<br />
b.	Discount on a U.S. Coast Guard-Approved Physical . This Discount applies to testing provided by the Family MedCenter, located near NMI in Fairhaven, Massachusetts;<br />
c.	Reduced Cost of Lodging.  Members attending NMI courses receive a reduced lodging rate of $50 per night at the Fairhaven Hampton Inn (www.lafrancehospitality.com). The Discount is subject to room availability. Member will be notified prior to the Member’s Renewal Date of an increase or decrease in the reduced lodging rate;<br />
d.	Assistance with TWIC and the U.S. Coast Guard license application process.  This benefit is offered to Members in attendance at NMI’s campus who would like assistance completing applications and scheduling appointments.  License applications submitted to NMI by mail or e-mail do not qualify. Career counseling is available for an additional fee;<br />
e.	Electronic Mariner Credential Recordkeeping and License Renewal Alert Service. NMI will electronically store each Member’s license and training documentation, such as the Member’s license, endorsements, z-card, physical and drug test results, and training certificates.  Reminders will be sent to the member’s email address when credentials are up for renewal;<br />
f.	Newsletter.  Member receives a complimentary copy of the The Northeast Mariner;<br />
g.	Computers and Reference Material.  To assist with self-study, Member receives access, at no cost to Member, to NMI study computers and on-site reference materials;<br />
h.	Apparel Discount of 10% on NMI apparel.  Member receives a 10% discount on apparel available through NMI over $15.<br />
i.	Additional benefits may be added as the program grows.<br />
9.	Publicity and Promotion.  NMI may include the Member’s name or photograph in press releases, websites or other collateral related to MET Co-Op after Member executes this Membership Agreement.<br />
10.	Member Warranties and Representations.  Member warrants and represents that it has authority to enter into this Membership Agreement and that it has not and will not make any representations and warranties for or on behalf of NMI or MET Co-Op.<br />
11.	Disclaimers of Warranties.  ALL INFORMATION, SERVICES AND GOODS, IF ANY, PROVIDED TO ANY MEMBER UNDER THIS MEMBERSHIP AGREEMENT BY NMI, DIRECTLY OR THROUGH NMI, IS PROVIDED SOLELY ON AN “AS-IS” BASIS, WITHOUT WARRANTY OF ANY KIND, INCLUDING, WITHOUT LIMITATION, THE IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, NON-INTERFERENCE AND NON-INFRINGEMENT.<br />
12.	Limitations of Liability.  NMI SHALL NOT BE LIABLE TO MEMBER OR ANY THIRD PARTY FOR SPECIAL, CONSEQUENTIAL, INCIDENTAL OR DIRECT DAMAGES, HOWEVER CAUSED, AND WHETHER BASED ON CONTRACT, TORT (INCLUDING NEGLIGENCE), PRODUCTS LIABILITY OR ANY OTHER THEORY OF LIABILITY, REGARDLESS OF WHETHER MEMBER WAS ADVISED, THAT EXCEED THE AGREGATE AMOUNT OF DUES PAID (OR DUE IF NOT PAID) BY MEMBER IN THE TWELVE (12) MONTHS PRIOR TO THE ACCRUAL OF THE CLAIM.<br />
13.	Assignment. Member may not assign, delegate or transfer this Agreement, or its rights and obligations hereunder, without the prior written consent of NMI, which NMI may give, withhold or condition in its sole discretion. Any attempted assignment without the prior written consent of NMI is void. This Agreement shall be binding upon, and inure to the benefit of, the parties and their permitted successors and permitted assigns.<br />
14.	No Agency. This Agreement does not create an agency, joint venture or partnership between the parties.<br />
15.	Choice of Law; Venue.  This Agreement shall be construed in accordance with and governed by the laws and venue of the Commonwealth of Massachusetts, applicable to contracts entered into and performed wholly within such state, without regard to principles of conflicts of law. Each party consents to the exclusive jurisdiction of the federal or state courts of Bristol County, Massachusetts over any dispute arising herein.<br />
16.	Entire Agreement; Waiver. This Agreement constitutes the entire agreement and understanding between the parties with respect to the subject matter herein and supersedes all prior, contemporaneous or other oral and written statements, representations or agreements by or between the parties. No changes or modifications of this Agreement shall be valid unless the same is in writing and signed by the parties. The failure of either party to enforce its rights under this Agreement at any time for any period shall not be construed as a waiver of such rights.<br />
17.	Counterparts. This Agreement may be executed in any number of counterparts with the same effect as if all parties had signed the document. Execution by facsimile or internet has the same force and effect as execution by original signature.<br />

I agree to follow all policies and procedures of the MET Co-Op and agree to the MET Co-Op terms and conditions outlined herein. I acknowledge that I have read and understand the terms of this agreement prior to joining MET Co-Op.</p>

<p>Launched in 1986 in New Zealand and named for the heroine in Paul Gallico&#8217;s book, The Snow Goose, the brigantine Fritha was built by traditional methods to unparalleled standards of excellence in materials and craftsmanship.  </p>
<p>The Fritha&#8217;s home waters of Southern New England offer some of the most beautiful scenery and the best sailing in the world.<br />
Interesting ports and anchorages including Buzzard&#8217;s Bay, the Elizabeth Islands, Martha&#8217;s Vineyard, and Nantucket are only a few hours&#8217; sail from Fritha&#8217;s home port of Fairhaven, Massachusetts.<br />
The Fritha can also meet you in places outside of her home waters as well.</p>

<p><strong>2010</strong> &#8211; Medical Care Provider   Aug 10 &#8211; 12<br />
<strong> Medical Care Person in Charge Course </strong>  Aug 13 &#8211; 20</p>
<p>MEDICAL CARE PROVIDER – a 24 hour program designed to enable the participant to meet the requirements of STCW 95 Ch VI, Section A-VI/4-1 Table A-VI/4-1. Upon completion the participant will be able to demonstrate a higher level of competency to take action upon encountering an accident or medical emergency as required by the code.</p>
<p>Through instructor directed presentations, video programs, and practical sessions, the program presents topics including Immediate Action to be taken in an Emergency, Body Structure and Function, Toxicological Hazards Aboard Ship, Medical Care of Rescued Persons, Cardiac Arrest, Burns, Fractures and Dislocations, Spinal Injuries and Immobilization, Administration of Medications, Maintaining a Healthy Environment Aboard Ship, and Use of Medical Advisory Services.</p>
<p>This course is a prerequisite for Medical Care Person in Charge.</p>
<p>MEDICAL CARE PERSON IN CHARGE – a 42 hour course aimed at providing the participant with the knowledge and skills needed to meet the requirements of STCW 95 CH VI, Sect A-VI/4-2 Table A-VI/4-2. After successfully completing this program the participant will demonstrate competence to provide medical care to the sick and injured while they are on board and to participate in coordinated schemes for medical assistance to ships.</p>
<p>In this program topics introduced in the Provider program are presented in a greater detail with emphasis on decision making. This course also provides training in basic nursing care, infectious and tropical diseases, dental injuries, ENT care childbirth and women’s health issues, disease prevention, wound closure. Basic skills in IV and injection of medication are also introduced in this program.</p>
<p>In order to be eligible for this program the participant must have completed a Provider level course within the previous 12 months.</p>

<p class="MsoNormal" style="margin: 6pt 0in; line-height: normal;"><span style="font-family: ">NMI’s sponsorship of a unique 12 week international program began in April, in Bermuda. NMI staff, local community members and several families from Bermuda, along with Bermuda officials, attended this milestone event in these young adults’ lives.</span></p>
<p class="MsoNormal" style="margin: 6pt 0in; line-height: normal;"><span style="font-family: ">Known as M.A.T.E, <strong>M</strong>aritime <strong>A</strong>pprentice <strong>T</strong>raining and <strong>E</strong>mployment program, eleven young adults from Bermuda were engaged in a maritime education program that centered on maritime training along with traditional life skills that sought to enable them to seek entry level maritime employment in addition to the possibility of higher maritime educational opportunities. The students attended the following NMI courses: Basic Safety Training, Able Seaman, Proficiency in Survival Craft, Operator of Un-inspected Passenger Vessels, Master 100 ton upgrade, Radar Observer and Bridge Resource Management. Several students have already been placed in maritime schools in locations such as Southampton England and locally here in New England. Several students  returned directly to Bermuda at the end of the program while others transitioned to Bermuda’s tall ship <em>Spirit of Bermuda</em> for additional training and sea time. After some well deserved time off, students will be making applications for employment in Bermuda and around the globe. NMI will continue to support their interest in a maritime career though maritime career counseling.</span></p>

<p>According to <a href="http://www.northeastmaritime.com/maritime/eric-r-dawicki-president/">Eric Dawicki </a>, CEO of Northeast Maritime Institute (<a title="Linked In Profile" href="http://www.linkedin.com/pub/a/588/16a">LI</a>), &#8220;The recent accident in Los Angeles could have been avoided if our system had been in use.&#8221;<br />
The system uses video cameras placed on the interior and exterior of a train cab and the images are then transmitted as live video feed to off-site locations. This allows viewing of the activities in the cab area of the locomotive. Key information of speed and other factors that the engineer should be observing can be remotely monitored. An alarm signals when locomotive are about to collide. All of this is done in real time using cellular or satellite technology. Train access is restricted to authorized individuals only. Video feed can be monitored anywhere there is access to the internet and security protocols are met.</p>
<p>&#8220;The technology we have developed to monitor the interior and exterior of train cabs using video feed in real time is groundbreaking. The recent accident in Los Angeles could have been avoided if our system had been in use. Using our system, the engine cab compartment area could have been monitored by an experienced team of engineers from a remote location. Through the creation of a team, coupled with a collision alarm we have integrated into the system, the deaths in California would have been avoided,&#8221; said Dawicki. &#8220;The technology works. By putting technology in the hands of experienced and well trained people, even the most adverse situations can have positive results.&#8221;</p>
<p>Sid Martin, Director of Technology (<a title="Linked In Profile" href="http://www.linkedin.com/pub/0/5a0/649">LI</a>), commented that &#8220;the system can be deployed quickly without disrupting normal rail operations.&#8221; The system, installed by NMI as a prototype in the field, has proven to be an excellent proactive defense. A patent is pending.</p>
<p>While safety is often overlooked in budget discussions, Northeast Maritime Institute thinks its system will be an easy sell to companies. &#8220;The product and its safety features speak volumes,&#8221; noted Martin.</p>

This course does not satisfy the training or assessment requirements of Section A-II/4 and Table A-II/4 of the STCW Code for Ratings Forming Part of a Navigational Watch. </span></span></p>
